Thank you for choosing our office to provide you with dental care. We value our relationship with you and believe that the best relationships are based on understanding. These are the explanations of the options for payment we offer.

1. Payment

Payment is due in full by cash, personal check, or charge card at each appointment as services are rendered. We accept Master Card, Visa, American Express, and Discover. We offer an extended payment plan thru Care Credit© (subject to credit approval)

A charge of $30.00 will be assessed on checks returned for any reason.  After two incidents of returned checks we will no longer be able to accept checks.

2. Dental Insurance

Dental insurance is a contract between you or your employer and the insurance company. We cannot influence what percentage of fees or services your insurance will cover. Your benefits are determined by the policy you or your employer purchase.  We will submit your claim to your insurance as a courtesy to you or provide you with the paperwork and information necessary for you to file your claim. Any or all reimbursements will be paid to the subscriber.

In the event that we receive a check from the insurance company we will VOID the check and send it back to your insurance company asking them to re-issue the check to the subscriber.

3. Pretreatment Authorization

Some insurance companies request an estimate of the work to be done and the fees to be charged before determining their benefits to you. We will be glad to send necessary documents to your insurance company for you or give you an estimate of necessary treatment codes and our fees if you wish to contact your insurance company on your own.

Please remember any pre-authorization or verbal quote from your insurance company is not a guarantee of benefits.

The scheduled appointment is reserved specifically for you. Any changes in this appointment affect many patients. If a cancellation is unavoidable please call our office at least 24 hours in advance so that we may give that time to another patient. If you have a Monday appointment and need to reschedule please contact our office no later than the Thursday before your appointment.

We strive to see all our patients on time for their scheduled appointment.  There are times when our schedule is delayed in order to accommodate an injured patient or an emergency. Please accept our apology in advance should this occur during your appointment. We will do the exact same if you are in need of emergency treatment. We will always keep you informed if we happen to be running late for your appointment.

Please plan to arrive 10 minutes before your scheduled appointment.  This will allow time for parking and to complete any additional paperwork required. Broken or missed appointments affect many people. If two broken or missed appointments occur or two cancellations without 24 hour notice, our office reserves the right NOT to schedule any subsequent appointments.

For your comfort and the comfort of our patients we respectfully request no cell phone use in the office.
