First Visit To Our Chapel Hill NC Office
An evaluation and discussion of your treatment options will typically be part of your initial session. The consultation can typically be followed by the beginning of the treatment. A complex medical history or treatment plan, however, will call for an assessment and a second session to administer treatment on a different day.
At the time of your consultation, kindly provide the following information to help us:
- If relevant, any x-rays taken by a prior dentist.
- Current medication list
- On the day of your initial appointment, kindly bring your dental insurance card. Time will be saved, and we will be able to assist you in processing any claims.
Important: A parent or legal guardian must accompany any patients under the age of 18.
If you have a health issue that should be brought up before treatment (such as diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or are currently on any drugs, please let the office know (i.e., heart medications, aspirin, anticoagulant therapy, etc.).
X-Rays
To get x-rays from your prior dentist before your initial visit, we will ask you to sign a records release form before your appointment.
